Provides staff with direction when reporting a work-related injury or injury on work premises

The intention of this document is to assist staff with reporting a work-related injury or injury on work premises

Procedure

  1. After an injury has a occurred, the employee needs to contact their immediate supervisor as soon as possible. The supervisor will need to report the injury to Human Resources within 24 hours.
  2. The employee should provide their supervisor with how the injury occurred, where it occurred, when it occurred, and who (if others) witnessed the injury. If urgent care was necessary, the employee needs to report where the care took place.
  3. The supervisor and the Library Administrative Office will then work with Human Resources to supply the necessary documentation on behalf of the employee.
