How to Change Your Legal Name for Faculty and Staff

These directions are for changing your legal name on record with Valparaiso University.

Step-by-step guide to updating your legal name

Follow these steps to provide an updated legal name. Access the electronic Change of Legal Name Form using this link: https://forms.valpo.edu/machform/view.php?id=59641

  1. Enter an effective date for the change using the calendar icon and selecting a date.                                                                                                                                                                                                                                                     

  2. Enter your first and last name.

  3. Enter your Valpo email address.

  4. Enter your full new name.

  5. Enter your preferred first name for email, if applicable.

  6. Upload Documentation (Driver's License or Social Security Card).

  7. Click the continue button.

  8. The Signature screen will now appear.

  9. Select whether you would like to add your signature by drawing or typing by choosing the draw or type option.

  10. If you select draw, simply draw your signature into the signature box using your mouse or laptop touchpad.

  11. If you select type, a box labeled Full Name will appear. Simply type your full name into this box.                                                                                         a

                           

  12.  Answer the Spam Protection question.

  13. Click the continue button.                         

  14. The Review Your Entry Screen will now appear.                                                                                                                                                           

  15. Review the information to ensure accuracy, if all the information is correct click the Submit button. If any of the information is incorrect, you can click Previous to navigate through the previous pages to make any necessary corrections.

  16. Once you click the Submit button, the Confirmation Screen will appear.                                                                                                                   

  17. You will receive a confirmation email with a copy of your submission, informing you that your Change of Legal Name form has been submitted to the Office of Human Resource Services.
