Reflection on goals from previous year
Briefly summarize your activities for the last year. Describe progress made toward achieving goals listed last year and any new ones added during the year. Explain what was completed, what was not and why. (Inability to complete some goals does not necessarily mean a negative evaluation.) In future, we may ask you to explain how your goals support the mission of the library and how they relate to specific library goals and objectives.

Productivity and reliability
Briefly assess your work performance. How well do you do your job? Can the library rely upon you to perform your assigned duties fully and in a timely fashion?

Initiative, creativity and problem-solving
Briefly assess your ability to take on new challenges, suggest improved ways of doing things and work out possible solutions to problems. What did you do this year to help the library or the University within the realm of your regular duties or beyond them? Note specific examples as appropriate.

Professional growth
Briefly assess your professional growth. What have you learned this year that helps you perform your duties better?

Teamwork and communication
Briefly assess your ability to work well with others. Do you support other workers when the need arises? Do you communicate and share information appropriately. This includes reporting problems promptly and documenting procedures as needed. Note specific examples as appropriate.

Describe what the Library administration or your supervisor can do to support you and make you more effective in your work

In future, we may ask you to explain how your goals support the mission of the library and how they relate to specific library goals and objectives. Your goals do not need to address the mission specifically, nor need they relate to all library goals. Choose those that most closely relate to your specific job
