Before you start your work, it's crucial to complete the New Student Hire Employment Paperwork Packet.
Step-by-step guide
Below is a breakdown of the forms included in the packet:
W-4 Form:
- Complete Steps 1-5. If needed, instructions can be found here.
State of Indiana WH-4 Form:
- This form is mandatory even if you do not reside in Indiana.
- More instructions can be found here.
Certificate of Residence WH-47 Form:
- Only required if you live in Kentucky, Michigan, Ohio, Pennsylvania, or Wisconsin.
Direct Deposit Agreement:
- Use your bank's routing number and your account number, if unsure of these numbers, contact your bank.
Employment Eligibility Verification Form I-9:
- Complete Section 1 only.
- For Section 2, email copies of approved documents and schedule a short video interaction.
- Copies sent to HR must include front & back if two-sided.
- Original documents must be presented during the live video interaction.
- Choose ONE document from List A OR one from List B plus one from List C.

- If submitting two documents, ensure they are from different lists, one from B and one from C.
- International Students: Use List A Section 5 (Foreign Passport and Form I-94).
Submission Details:
Once completed, send a copy of the packet and copy of chosen document(s) for Section 2 of Form I-9 to human.resources@valpo.edu. You may also submit the completed packet and approved documents in person to the Office of Human Resource Services during regular office hours: Monday-Friday 8am-12pm & 1pm- 4pm.
After submitting packet and documents through email, use this link to schedule a live video interaction with an HR representative. During the interaction, present the same original document(s) submitted as copies in the previous step. You will be set a link for the video meeting.
*Note to International students* Ensure you have a Social Security Number on your documents before submission. Contact Andrew Knox, International Programs, for more assistance on obtaining a Social Security Card.
Please note the following:
- A student cannot begin working until this has been completed and their employment paperwork has been processed.
