Statement
Students planning to take MGT-395 (Internship Experience in Business, Administration I) complete 3 forms prior to their internship and 1 post-internship form. Additionally, the employer completes 1 pre- and 1 post-internship form. When the forms are completed they route automatically to Perceptive where a folder is created for each student. Once all required forms are in a student's folder, the folder routes to a queue for permanent recordkeeping. The Advising and Practical Experience Coordinator oversees this proces. The following instructions apply to the Coordinator and COB faculty or staff that may assist with internship form management.

5. Next, the Coordinator will scan & link, and route forward subsequent forms for the student. These will land in the COB - Student Documents queue. When all of the required forms arrive in the student’s folder, the folder and all of its contents will automatically route to the COB - Student Docs Rec’d queue. The The Advising and Practical Experience Coordinator will be automatically notified that the required documents have arrived in the queue. The coordinator will reviews review forms, grant any necessary permissions permission for the student to register for the cousrse course and then last, route the student folder forward to the COB - Current Internships queue.

POST-INTERNSHIP
6. Student completes Intern Self-Evaluation form. Completion of this form auto sends the Employer Intern Evaluation form to the employer.
7. Employer completes the Employer Intern Evaluation form.
8. Coordinator or COB Admin. will scan & link and route forward these post-internship forms in Perceptive. These will route to the student’s folder in the COB - Current Internships queue. Now that all forms have been received, the folder will automatically move to the SAF - Document Storage queue.
