Notes in Slate Student Success

Any staff member may submit a note for a student. Here are the steps to create a note:

Step 1: Begin a note from any of the Add a Note buttons

Step 2: Select a note type - Early Alert, Check-In, or Meeting Note

Early Alert - a concern that needs follow-up either by the student's primary advisor or another support unit. This note sends an email to the student's primary advisor, you may also select whether to notify the student and/or make a referral to a support unit.
Example: "Ben Beacon has missed the last week of my class with no response to email."

Check-In - a note of kudos, mild concern, or general check-in. A copy of this note is sent to the student.
Example: "Great job on yesterday's quiz!" or "Missed you in class today, hope all is well."

Meeting Note - record of a meeting or interaction with a student for your or others' future reference. You may select whether to notify the student and/or make a referral to a support unit.
Example: "Met with Bella Beacon to discuss her graduate school and research interests in plate tectonics."

 

 


Step 3: Complete the Note Content

  • For an academic concern, select the type of concern (Assignments, Attendance, Engagement, Quiz or Test Score, Other)

  • Complete the Brief Description to add content to the note

Step 4: Select Notification and/or Referral Options (if Early Alert or Meeting Note)

  • Send a copy of notes to student? Select Yes or No

    • This will send a copy of the note, including referrals recommended, to the student

  • If a referral is needed, Select Yes, then select the support unit(s) for the referral

    • This will send a copy of the note to each of the support units selected

An Early Alert always notifies the student’s primary advisor
