Employee Tips for Completing a Performance Review

  1. Set a day and time to complete your performance evaluation. Try to find a time that will have minimal interruptions and allows you to focus.

  2. Locate your performance evaluation from last year to review last year’s goals. If you do not have a saved copy of your performance evaluation from last year, please check your email. A copy of your final performance evaluation was automatically emailed to you from no-reply@forms.valpo.edu, titled HR – Performance Evaluation Supervisor.

  3. Think back over the past year. What went well? What could have gone better? What are some highlights and achievements of the last year?

  4. Think ahead. What do you want to accomplish over the next year? What new things are you looking to learn or skills do you want to improve? Identify areas of opportunity that can be improved over the next year.

  5. Review the performance evaluation ratings listed below. When deciding the rating you want to select, be honest with yourself and select the rating that most closely reflects your work performance consistently. When rating yourself as a (5) VU Exemplary EE or as a (2) Development Needed, be prepared to include specific examples explaining why you selected this rating.

(5) VU EXEMPLARY EE: This rating is reserved for those few staff who made a profound impact this year.

Consistently went way beyond core job responsibilities.

Exceeded all expectations and goals, making a profound impact on the school/department and University beyond what was planned.

Demonstrated VU values in an exemplary way and modeled the values for others. Ready for expanded or new responsibilities.

(4) EXCELLING: staff that made a significant impact this year.

Went above and beyond core job responsibilities.

Exceeded expectations and goals, making a significant impact on the school/unit beyond what was planned.

Demonstrated VU values in an exemplary way.

May be ready for new or expanded responsibilities.

New hires or newly promoted progressed significantly faster than expected.

(3) ACHIEVING: staff that made a solid impact this year.

Fulfilled core job responsibilities.

Achieved goals, making a solid impact on school/unit.

Demonstrated VU values consistently.

Showed personal growth.

(2) DEVELOPMENT NEEDED: made a limited impact this year. There is development needed in one or more areas under review. This rating is appropriate when one or more of the following statements apply:

Did not consistently fulfill core job responsibilities.

Did not consistently meet expectations and goals.

Did not consistently demonstrate VU values.

Did not show much personal growth.

(1) CORRECTIVE ACTION: failed to improve despite ongoing efforts to address performance issues.

There is a box at the bottom of each page to “save progress and resume later.” You will be provided a link to access your incomplete evaluation and continue where you left off. Please be sure to save a copy of this link so you can access your evaluation later.
