- Provide at least one Emergency Contact in Self-Service
- Payment (covered by student fees for full-time students)

- If you need to pay for your membership (faculty, staff, part-time student, etc.) complete the Payment Form
- Full-time students' membership to the Fitness Center is included in Student Fees for the Fall and Spring Semesters.
- Summer student memberships are available for a nominal fee.
- Log into Self-Service using your Valpo username and password
- Click on your username in the upper right-hand corner
- Select Emergency Information
- Click Add New Contact
- Complete at minimum a name and phone number for one contact
- You will not be able to save a new contact without a phone number
- Click Add New Contact
- Return to your username, then select Required Agreements
